Nei pazienti portatori di defibrillatore, che vadano incontro a frequenti interven- ti dellapparecchio, lamiodarone pu migliorare la prognosi quoad vitam e soprattutto quoad valetudinem . Va infine ricordato che, a causa dellelevato numero di effetti collaterali, lamiodarone nei soggetti con cuore sano ed aritmie ventricolari non dovrebbe essere utiliz- zato come farmaco di prima scelta. . . .  Lamiodarone somministrato per via endovenosa ha dimostrato la sua efficacia antiaritmica anche in pa- zienti ospedalizzati. In uno studio statunitense, 342 pa- zienti con tachicardia ventricolare incessante o ricor- rente e/o fibrillazione ventricolare sono stati randomiz- zati a ricevere tre dosaggi diversi di amiodarone (125, 500 o 1000 mg) nelle prime 24 ore. In tutti i pazienti si osservata una riduzione del numero di episodi rispet- to alle 24 ore precedenti il trattamento. I pazienti che avevano ricevuto 1000 mg avevano una riduzione del numero degli episodi maggiore rispetto a quelli trattati con 125 mg (p = 0.043) 10 .
