Identify and manage pre- hypertension and hyper- tension. 2. Counsel patients in lifestyle modification. 3. Prescribe medications appropriately when required, especially in patients with co-morbidities such as diabetes, coronary heart disease, and chronic kid- ney disease. 4. Recognize factors contributing to poorly controlled hypertension and be able to apply suggested actions to overcome them. Accreditation The DOHMH is accredited by the Medical Society of the State of New York to sponsor continuing med- ical education for physicians. This continuing med- ical education activity is designated for a maximum of 1.5 hours in Category One credit toward the AMA/PRA (Physicians Recognition Award). Each physician should claim only those hours of credit that were spent on the educational activity. Participants are required to submit name, address, and professional degree.
